FMLA updated rules 3/2008

Family Medical Leave will continue to be administered by FMLASource on behalf of Momentive Performance Materials.
Employees must contact the FMLASource immediately www.FMLASource.com to initiate their leave request and to have questions answered concerning the employee's responsibilities in the process at their toll free number 1-877-432-3652. Employees can click on the tab "Request a Leave" to start a request for FMLA. Employees can also contact FMLASource and ask questions about FMLA either by calling the phone # above or click on the link to send an email to fmlacenter@fmlasource.com.
This process will continue to ensure that all FMLA cases are properly approved and administered and ensure that qualified claims are filed.
Employees that have an ongoing medical condition or family member that requires their care must contact FMLA.
The employee may also use the FMLASource web site for general information .
Remember:
If an employee uses FMLA (absent/tardy/leave early) the employee must call FMLASource in addition to informing their Leader/Manager. i.e. coming in late, they must call; going home early, they must call, etc.
