2011 U.S. Flexible Spending Account Claims Filing Deadline is March 31

For associates who were enrolled in the company sponsored healthcare or dependent care Flexible Spending Plans in 2011, the deadline is nearing for you to file final claims for reimbursement for the 2011 plan year. The filing deadline is March 31, 2012.

Claims must be postmarked by March 31, 2012 to be considered for reimbursement.

You may mail or fax your FSA claims to ADP at:

If you mail your request for reimbursement, claims must be postmarked by the U.S. Postal Service by March 31, 2012. If you fax your request for reimbursement, the fax must be received in ADP’s offices by midnight on March 31, 2012. Be sure to save your fax confirmation as proof of submission. No late submissions will be processed.

Please complete your claim forms thoroughly and accurately, sign and date them and attach the appropriate level of documentation required for processing. A list of acceptable proof is outlined on the flexible spending account claim forms.
